"Canadian scientists have found a new link between two major metabolic diseases: diabetes and steatotic liver disease, commonly known as fatty liver disease." "Published in the scientific journal Diabetes, research into the two diseases by Université de Montréal medical professor Jennifer Estall and her team at the Institut de recherches cliniques de Montréal (IRCM) / Montreal Clinical Research Institute identifies potential new biomarkers and targets for therapy." "The study revealed an abnormally high presence of proteins called sIL-6R and sgp130 in the blood of patients with steatotic liver disease, and even higher levels in those with diabetes." "The team was able to establish that high sugar and lipids – which are found in diabetes – trigger different types of liver cells to produce these proteins, which are involved in controlling inflammation." "Most strikingly, the study revealed that sgp130 concentration in bloodstream is a very good indicator of liver damage in both men and women, suggesting this protein could also be a good circulating biomarker of liver disease." #diabetes #NAFLD #MASLD

Congratulations for identifying a new potential biomarkers both for diabetes and liver steatosis. By short reviews of sgp130, the endogenous IL-6 inhibitor, the interaction between IL-6/sIL-6R and sgp130 has unique behavior compared to other crosstalks. Usually the soluble forms and membrane bound forms of the proteins show quite different functions and fates of their signaling pathways. Another interesting aspect of this research brings certain assumption that the increases of IL-6/sIL-6 complex induces sgp130 for the next stage prevention.

🌟 Exciting News! 🌟 I am thrilled to share our latest publication "Transfer of modified gut viromes improves symptoms associated with metabolic syndrome in obese male mice." Our research demonstrates that modifying gut viromes can alleviate metabolic syndrome symptoms, offering new insights into potential treatments for obesity-related conditions. 📊 Key Findings: 🔍Mice administered a modified FVT (nearly depleted for eukaryotic viruses) exhibit enhanced blood glucose clearance but no weight loss 🔍The unmodified FVT improves liver pathology and reduces the proportions of immune cells in the adipose tissue with a non-uniform response. 🌟 Implications: Optimizing approaches could lead to the development of safe bacteriophage-based therapies targeting metabolic syndrome through GM restoration. Inflammatory skin diseases, especially psoriasis, have long been linked to obesity as a significant risk factor. In our study, we delve into the intricate mechanisms behind this association. Our research highlights how diet-induced obesity disrupts a unique subset of PPARg+ Tregs in the skin, triggered by fatty-acid-induced lipotoxicity. This disruption leads to the loss of control over IL-17A+ gd T cell-mediated inflammation, resulting in the exacerbation of psoriatic conditions. 🌟 Why is this Important? Understanding the role of skin-resident PPARg+ Treg cells in psoriatic inflammation sheds new light on potential therapeutic targets. I'm delighted to share our findings published today in Nature Communications Medicine, a culmination of three years of dedicated research on novel markers for predicting #cardiovascular disease in #type2diabetes. As a co-chair and senior author alongside Dr. Maria F. Gomez at Lund University in Sweden and Dr. Ronald Ma at the Chinese University of Hong Kong, this research has been a remarkable journey. Our global team, part of the ADA/EASD Precision Medicine Diabetes Initiative, involved 23 experts from 11 countries, and aimed to identify novel biomarkers, genetic markers, and risk scores to improve cardiovascular risk prediction beyond traditional factors for patients with type 2 diabetes. Our systematic review of nearly 9,400 studies led us to some intriguing discoveries: - Among hundreds of biomarkers studied, only 13 showed a significant improvement in prediction accuracy. - NT-proBNP stood out as a powerful predictor, not only for heart failure but potentially for various cardiovascular outcomes. - In the realm of genetics, of the 79 markers studied, some, like the isoform e-4 in APOE and the Genetic Risk Score for Coronary Heart Disease (GRS-CHD), demonstrated moderate predictive ability. - Our extensive review of 27 different risk scores across 47 studies highlighted their varying effectiveness. There was relatively low discrimination seen using UKPDS and Framingham Risk Scores when validated in diverse populations. This finding is particularly important as it emphasizes the need for tailoring risk assessments to specific populations. These findings underscore the potential of personalized medicine in improving care and outcomes, but also highlight the need for more rigorous research in diverse patient populations. In The Lancet: Intratumoral Microbiome Differences In Young vs. Old Colorectal Cancer Patients - A Clinical Study #Mysummary Researchers at the Cleveland Clinic have discovered significant differences in the tumor microbiome profiles of young-onset colorectal cancer (CRC) patients compared to the average population with the disease. They analyzed the microbiome of fresh surgical specimens from 136 patients diagnosed before age 50 (Young) and 140 over age 60 (Old) using 16S sequencing. The data showed several interesting revelations: 1> Tumors of the Young group have a higher microbial diversity 2> Key genera enriched in Young CRC patients were: Akkermansia, Bacteroides 3> Key genera enriched in Old CRC patients were: Bacillus, Staphylococcus, Listeria, Enterococcus, Pseudomonas, Fusobacterium, Escherichia/Shigella 4> Akkermansia abundance was positively linked with improved overall survival. #Mythoughts For years, the cancer research community has centered on Fusobacterium as the singular pathogenic culprit responsible for colorectal carcinogenesis. However, this Cleveland Clinic study reveals new microbial leads—Akkermansia and Bacteroides are rising stars that could potentially be leveraged to stem early-onset colon cancer. But perhaps more significantly, the age demographic itself seems to be shifting dramatically downward for CRC onset. While high-risk lifestyle choices are likely fueling this surge in young adults, we cannot ignore the possibility of generational shifts in overall gut microbiome composition increasing susceptibility early in life. Clearly, we need to expand our thinking and explore multifaceted microbiome-modulating prevention strategies. Whether through personalized diets, probiotics, prebiotics, or even fecal transplants to rehabilitate the gut, the time has come to test integrative protocols to protect those at risk and decisively intervene against this exacerbating crisis of early-onset disease. A new article highlights the impact of oral microbial imbalances on #gut health and systemic disease. The oral microbiome, which contains over 700 species of #bacteria, is linked to conditions like periodontitis, dental caries, and atherosclerosis. Imbalances in the oral microbiome are associated with conditions like rheumatoid arthritis, colorectal cancer, and intestinal tumorigenesis. Aggregatibacter actinomycetemcomitans, which translocates to the gut in mice when fed a high-fat diet, can exacerbate insulin resistance and non-alcoholic fatty liver disease. #Streptococcus mutans, a cariogenic bacteria, can cause systemic inflammation when ingested and introduced to the gut. #Prevotella intermedia, linked to atherosclerotic #cardiovascular disease, can be aspirated into the lungs and lead to aspiration pneumonia. As practitioners, it is crucial to remember the oral-systemic connections and support #microbial balance through diet, prebiotics, and probiotics. 🔬🧫🧬 Impairment of epithelial barrier and induction of inflammation in the intestine underline a variety of pathologies in humans including inflammatory bowel diseases (IBD) as well as metabolic and neurological diseases. 👩⚕️ Intestinal microbiota and the host immune system are the key players determining intestinal homeostasis and inflammation. 🧬 Novel bio-therapies such as pre- / pro- / and post-biotics are extensively developed to aid maintaining and restoring the impaired balance between intestinal microbiota and host immune response. Their selection and validation is a complex process involving in-vitro and in-vivo models, while the former is lacking the complexity of the physiological system, the latter often fails reflecting the effect in humans. 👨🔬 The development of new pertinent and easy-to-use models is therefore needed. 🧫 The current Gut-on-a-Chip model recapitulates the key features of intestinal inflammation and enables an easy screen of novel (bio) therapeutics in a mid-throughput manner. Moreover, the use of donor derived immune cells and fecal material paves the way towards personalized medicine approach. 🧬 Future development will be focused on testing the effect of matched fecal microbiota and PBMC from IBD patients compared to healthy donors. Liver disease continued to make headlines this past year, with the renaming of non-alcoholic steatohepatitis (NASH) leading the charge. Now known as metabolic dysfunction-associated steatohepatitis (MASH), the rename has been decades in the making, as scientists and clinicians agreed than the previous name "trivialised the disease and was scientifically inaccurate". They go further: liver disease is incredibly complex, as are the patients and how they present. The old name did a disservice to this complexity and "was not really reflecting the underlying pathology" of the disease. This homogenisation has affected our understanding of liver and, in turn, impacted therapy development. It is hoped that the new name will help break this pattern; marking a major shift in our understanding of the complex drivers of disease and, ultimately, leading to a new era of liver disease therapies.
