Update/issue templates

Description of the Change

This PR converts the issue templates into issue forms to try and capture the desired input in a more user-friendly way (i.e., more precise, smaller text areas versus one giant text area). It also adds in links to the troubleshooting info in the readme in an attempt to hault some issue submissions that may be solved via self-service.

We might also want to consider customizing the security policy that's inherited from the .github repo.
