Sometimes social determinants of health get in the way of patient care. Take Tina for example. She's a 16-year-old Medicaid beneficiary who was diagnosed with asthma when she was four. Her home's air conditioning stopped working, and her mom started to notice that Tina required her rescue inhaler more often. She was referred to our team at CHESS as part of the new Healthy Opportunities Pilot. We referred her to a Home Expense Assistance program that covered the cost of a brand-new AC for her family. Smoking-related illnesses cost the nation more than $300 billion every year. But you can play a vital role in helping your patients quit smoking. You can help by: + Advising them to quit tobacco use and discussing how that helps improve overall health + Offering tobacco cessation counseling + Prescribing tobacco cessation medications + Connecting patients to additional resources + Continuing support after quitting to prevent relapse + Continuing routine tobacco use screenings even after cessation The U.S. Public Health Service recommends implementing the 5 A's shown below to treat tobacco dependence.
