Addiction Treatment Breakthrough! U.S. Congress finally drives CONTINGENCY MANAGEMENT (CM) forward in the new Omnibus Spending Bill, just resolved late last night in bipartisan agreement. Never before has the U.S budget process called for the Secretary of HHS and the Office of the Inspector General to resolve how to foster widespread adoption of CM. CM (motivational incentives) is the single most effective, best-evidenced, yet least utilized approach in substance use disorders treatment. It literally DOUBLES abstinence rates -- from opioids, stimulants, alcohol & nicotine. It's a simple formula: Better Outcomes = Lives Saved = Cost Savings = Less Stigma Now, the Senate & House are putting CM into the federal budget! Stimulant overdose deaths are skyrocketing, increasing by 317% from 2013-2019. Yet the federal government is stubbornly sticking to antiquated rules that hamstring one of the most evidence-backed treatments we have - contingency management (CM). More than 100 randomized controlled trials over 50 years have shown CM can double rates of abstinence from opioids, stimulants, alcohol, and nicotine compared to standard care. It's the only evidence-based treatment for stimulant use disorders. But current laws cap non-monetary CM rewards at just $75 per patient per year - a limit that has remained unchanged for decades despite rising costs. The National Council for Mental Wellbeing is calling on the Biden administration to increase this archaic $75 cap and exempt CM from outdated fraud and abuse rules treating these motivational incentives as illegal kickbacks. CM leverages positive reinforcement to encourage healthy behaviors and accountability for recovery - there's nothing illegal about that. The overdose crisis is disproportionately devastating Black Americans and rural communities. Easing restrictions on CM would improve health equity and access to this proven, life-saving approach. We cannot keep clinging to policies from the last century while overdoses claim over 100,000 lives per year. Stimulant use disorder (StimUD) rates are rising nationwide, yet no FDA-approved medications exist to treat meth, cocaine, and other stimulant addictions. This is an increasingly urgent public health crisis, but there is a solution. The one intervention that works in study after study, Contingency Management (CM), is recommended by ASAM and gaining attention nationwide, but rarely used due to regulatory barriers. However, DynamiCare Health is the only company to have received a favorable HHS-OIG advisory opinion. This opinion was referenced in the article, and addresses all necessary safeguards for CM. It's also important to note, DynamiCare Health’s telehealth program substantially differs from the manual, data entry-based CM pilot now underway in California which "...[CM] tends to stop working for people once they stop attending [treatment]" Because our model is telehealth-based, engagement and outcomes have consistently stayed high, whether or not people attend treatment.
