One Community Health is seeking a dedicated School-Based Outreach Coordinator (SBOC) to join our School-Based Health Services team. This grant-funded position offers the unique opportunity to travel to multiple OCH clinic locations and community school events. While there is a high likelihood the grant funding for this position will be renewed on June 1st of 2025, it is not guaranteed.

The role is a full-time position with a flexible schedule between clinic locations and school-based events. The schedule for this position will be at the Dufur School-Based Health Center for two days a week, Mondays & Thursdays. Other weekdays will be focused on outreach which includes enhancing relationships between the school staff, local public-school officials, student-patients, and their families across Hood River and Wasco counties. The SBOC will play a key role in identifying and addressing barriers to accessing preventative and acute services at our SBHC, collaborating closely with the Director of School-Based Health Services to develop and implement creative solutions. Minimum Qualifications

  • High school graduate or G.E.D.
  • Skilled in prioritizing and organizing work according to multiple and competing timelines
  • Capable of meeting timelines for team and organizational goals with safety and high quality
  • Strong written and oral communication skills
  • Proven relationship management skills with multidisciplinary teams in a diverse, multicultural environment
  • Initiative and good judgment to resolve problems and challenges
  • Effective in teamwork and comfortable working with ambiguity
  • Able to inspire trust and confidence and communicate effectively with internal and external audiences
  • Independent worker with the capacity to manage multiple projects while maintaining a high degree of professionalism and confidentiality
  • Knowledge of local community resources
  • Proficient in computer skills, including Microsoft Office applications (Word, Excel, Outlook), Epic, and other systems
  • Knowledge of and compliance with HIPAA regulations
  • Understanding of customer service and workplace safety
  • Effective in both team environments and self-directed tasks
  • Willingness to travel for events and between clinic locations, as needed
  • Willingness to work occasional weekends and evening events, as needed
  • BLS/CPR Certified (must be completed within 30 days of hire)

Preferred Qualifications

  • Bilingual English/Spanish language skills
  • Reception and patient registration experience
  • Epic experience or other EMR
  • Knowledge of medical terminology
  • Experience in a health related or social service-related field
  • Post high school education in office/clerical coursework
  • Experience working with low income, multi-ethnic populations

Compensation

  • Hourly/Non-Exempt: Starting at $22.00 per hour, dependent on experience
  • Weekend Differential: This position receives base hourly pay along with an extra $10.00 for each hour worked on weekends (Saturday and Sunday)
  • Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish
  • Mileage Reimbursement: In accordance with OCH Company Policy.
