We’re excited to share that East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving #EastToronto, recently became one of the first OHTs to receive a Best Practice Spotlight Organization (BPSO) designation! 🎊 The BPSO designation recognizes health and academic organizations that successfully implemented best practice guidelines to optimize health outcomes. “This designation is the result of four years of hard work, building capacity among partners to implement best practice guidelines and share evidence-based practice and education materials across the different sectors of care,” says Margery Konan, Manager of Integrated Care at ETHP. “It’s exciting to be one of the first OHTs to do this work, and to be recognized for all of our efforts is really special.” Thank you to our partners and community members, for their hard work over the last four years to help us achieve this incredible milestone. As a part of our commitment to provide low-barrier care for youth and young adults, MGH has joined as a partner at two Youth Wellness Hubs Ontario (YWHO) sites in East Toronto. MGH is part of a team of organizations providing essential services at these sites for youth aged 12-25, such as mental health counselling, peer support, primary care and employment services. After being assessed in our Emergency and Diagnostic Imaging Departments, Ummema received a diagnosis that was completely unexpected – one that caused her care team at MGH to spring into action and provide her swift, life-saving care in the span of just a few hours. “We want to express our gratitude to the entire team that helped,” Ummema and her husband, Abdul, say. “It’s these people who are heroes and our angels on this Earth.” Ummema’s experience at MGH is one example of the collaborative and emergency care that the interdisciplinary teams provide every day. In this case, their quick thinking and teamwork allowed for a timely, coordinated effort that identified and addressed a potentially life-threatening condition in a patient – and resulted in a positive outcome for everyone involved.
